Medical Treatment Support Program – PAV-H

Project Summary:

This project aims to provide essential medical treatment support to vulnerable populations who lack access to necessary healthcare due to financial constraints or systemic barriers. By offering treatment assistance, medication, and follow-up care, PAV-H seeks to reduce morbidity and mortality related to common and chronic diseases.

Background:

Many individuals in underserved communities suffer from illnesses that go untreated due to poverty, limited healthcare infrastructure, and lack of awareness. Common causes leading to medical treatment needs include infectious diseases, chronic conditions (e.g., diabetes, hypertension), and acute emergencies. Without timely treatment, these conditions worsen, impacting quality of life and survival.

Objectives:

  • Facilitate access to essential medical treatment for at least 300 patients annually.

  • Provide medications, diagnostic support, and follow-up care for priority conditions.

  • Educate patients and families on disease management and prevention.

  • Reduce complications and hospital admissions related to untreated illnesses.

Activities:

  1. Patient Identification and Referral
    Collaborate with local clinics and community health workers to identify patients in need.

  2. Medical Treatment Provision
    Supply medications, perform basic diagnostics, and facilitate treatment plans.

  3. Health Education Sessions
    Conduct workshops on disease prevention, medication adherence, and healthy lifestyles.

  4. Follow-up and Monitoring
    Track patient progress and adjust treatment as needed to improve outcomes.
